Changing Contact Details
Our patients often move address, change their mobile phone or get a new email address without telling us. Please ensure you use the online change of address form to update your contact details.
OR come into the Surgery and inform us of your change in details OR advise us of your change in details when you make an appointment.
This is essential because we may need to contact you with results of tests or to refer you to another service for further care or advise you to make an appointment with the Doctor or Nurse.
